All required fields are to be completed before submission to the State Chapter CME contact at statechapterscme@asmbs.org<\/a>.<\/p>\n\n\n\n Download the ASMBS State Chapter Joint Providership CME Application and Activity Planning for AMA PRA Category 1 Credit<\/a><\/strong><\/p>\n\n\n\n All faculty\/speakers, planners, abstract reviewers, moderators, authors, co-authors and administrative staff participating in the continuing medical education programs sponsored by the ASMBS are expected to disclose to attendees any\/all relevant financial relationships related to the content of their presentation(s) in accordance with ACCME Guidelines for Commercial Support. It is the Non-Accredited State Chapter\u2019s responsibility to collect ALL disclosures and submit them to the ASMBS prior to planning.<\/p>\n\n\n\n ASMBS Financial Relationship Disclosure Form<\/a><\/strong><\/p>\n\n\n\n If an Application is provided at least 3 months in advance of the<\/strong> meeting, ASMBS can assist in collecting financial disclosures for speakers.
